Editorial

The XIV SLAMS Congress, held in Lima, Peru, was an outstanding meeting. Approximately 300 partipants attended the meeting, from all Latin America and the program was of great scientific level. We would also like to thank the industry for its support, who once again have joined our efforts.

During the congress, I had the honor of being elected as President of our Society for the next two years. I would like to thank all the previous Boards for their dedication to our society.

It is our goal for these next years to work hard on continuing education on Sexual Medicine, which will be one of our priorities, through the organization of courses and symposia, and also participating in congresses and meetings of related specialties. On the other hand, once again SLAMS committed to the organization of the next congress in the city of San Pablo, Brazil, in 2019.

I would like to thank SLAMS members for the trust and I guarantee that the SLAMS Board will work hard and that we are committed to achieve our objectives.

SMSNA Meeting - Report

This meeting characteristic is the in-depth treatment of the different pathologies involved in Sexual Medicine, both male and female. In a well-balanced scientific program, there is a combination of basic science, research, and of course medical and surgical treatment of the various sexual dysfunctions.

Among the topics addressed, we would like to highlight the following:

  1. Management of genial and scrotal pain (L. Levine). After recommending a detailed study of the multiple conditions that may cause the pain, and if found to be treated, the study mentions the microsurgical denervation of the spermatic cord as an alternative if the testicle is to be preserved. Before, it is essential to perform a therapeutic test of bupivacaine infiltration into the spermatic cord to determine if the patient may benefit from this procedure.
  2. Penile rehabilitation (J. Mullhall) has more supporters than critics. Most still use phosphodiesterase inhibitors as main alternative, and protocols were presented including their use between 2 weeks prior to 2 weeks after radical surgery, usually in low doses, increasing the dose near the 6 weeks. Then it must be determined if there is any response to the drug in order to decide to continue with the treatment or to begin with intracavernous therapy or other alternatives. It is important to note that the worst results in the patients' sexual performance are observed at 16 weeks after the surgery, and are probably the result of the corpus cavernosum "collagenization."
  3. Among the presented posters we would like to mention the paper on daily Tadalafil as new treatment option for Peyronie's Disease. Based on the research by Ferrini (2006) the poster mentions how it may prevent the formation of plaques in rats, and on the paper by Serefoglu (2011) with its antibiotic effect in patients who underwent radical prostatectomy. Although the paper mentions its potential utility, it only has 66 patients and it makes necessary a better patient follow-up and a higher number of patients.
  4. Management of erectile dysfunction and Peyronie's Disease (S. Wilson) has gone through different stages which include: Inflatable prosthesis plus plicature (Morey), tunic incision procedure plus implant (Ralph), implant plus "Scratch" (Perito), or prosthesis plus plaque resection and graft (Levine, Carrion, Kholer). Certainly, the "modelling" technique for penile curvature of 40 degrees or less continues to be a non-invasive and effective alternative for many patients undergoing prosthesis surgery.
  5. Dr. Steve Wilson mentioned some of the surgical tips, which years of experience have taught him: Do not recommend the performance of the dual implant of artificial urinary sphincter and inflatable penile prosthesis at the same time and possibly defer the prosthesis. Not getting to know the patient before the surgery will probably result in an unsatisfied patient; and placing ectopic reservoirs, after knowing very well its location in the space of retzius, may sometimes cause unexpected complications.
  6. Among the many messages related to the penile prosthesis surgery, it is worth mentioning that some of the few reasons that takes you back to the operating room are: Opening of the wound with early blood or purulent drainage, evident infection of the implant with pus, ischemia of the glans after the prosthesis implant or a perforated viscus.
  7. The ESSM (European Society of Sexual Medicine) held a conference block where they mentioned the many events that may occur after radical pelvic surgery, especially the role of cytokines and reactive oxygen species in neuropraxia, the loss of nocturnal erections related to the accessory pudendal arteries damage and cavernous hypoxia worsened by factors such as TGF beta, Endothelin 1 and reduction of PGE1. They also mentioned the possible role of low intensity shock waves in patients with pure vascular erectile dysfunction, with veno occlusion disorders and non-responders to oral drugs, neurogenic ED and diabetics, although there is a lack of long-term studies, with technical details related to the type of waves and protocol to be used.
  8. Testosterone and cardiovascular risk were also addressed, especially when studies suggesting their relationship were mentioned (Finkle 2014, Basaria 2010, Vigen 2013 , Xu 2013), but with an inadequate methodological design and with many other studies referring to the replacement therapy as a protective factor.
